BEFORE YOUR FIRST VISIT

List signs and symptoms that can be of importance on intake form.
List past medical history, previous treatments, current medications, and other important information that is important on intake form.
Please verify your own insurance benefits.
Arrive on time for your appointments.
Bring with you a copy of your insurance card
Wear loose comfortable clothing that will expose the body part I will be treating, and allow you to move freely.
